QC New Defect window automatically closes when attaching photo

Hi,

I am using QC11.52 and think we may have done or amended something in the workflow. If  we raise a defect during execution and attach a screenshot the defect pop-window automaitcally closes. Does anyone know what I should amend to resolve this.
