User form in ALM

Can we create a user form in ALM?

I'm trying to find solution for the below problem:

A user form  should be populated on click of 'Generate' toolbar option of an excel report, in Dashboard module. I should be able to select the Releases which are displayed on that user form. Once I click submit, it should generate a report based on the query i wrote in the Qyery builder.

Is it possible to create a user form and link it to ALM DB?
