Customizing the Check In/Check Out popup window for Version controlled projects for HP ALM 11.5

Hello All,

I have a requirement of customizing the content in Check In/Check Out popup window for Version controlled projects, in HP ALM11.5.

i.e. For a version controlled project, when we check out or check in an entity, a window pops up. In that window we enter our change comments. I want to change the content of the window using the workflow

I have tried the OTA guide, but was not able to find anything. Please point me in the right direction
